Overview
Two permanent Senior Occupational Therapist (OT) posts within the Independent Living Service. One post based in the main area of the Independent Living Service; the other in the Discharge to Assess function of the Independent Living Service. Both posts involve supporting hospital discharge, community follow-up, and specialist input for people with disabilities across Bolton, working closely with Community Therapy and Social Work teams.
Responsibilities
* Support safe hospital discharges and follow-up care in the community to prevent readmission.
* Carry out Care Act 2014 assessments and develop appropriate support plans.
* Apply relevant legislation (e.g., Mental Capacity Act 2005) in practice.
* Assess needs, provide equipment, and promote independence in collaboration with therapy and social work teams.
* Contribute to service development and improve user and carer experiences through person-centred planning.
* Support integrated care through collaborative working.
* Work on a 5 from 7 rota to include weekends and bank holidays, with a 7% enhancement for weekend work.

We are looking to recruit an enthusiastic and motivated individual who has a passion for working with adults, understanding their needs, aspirations and supporting them to enhance their quality of life and promote independence. You should be able to develop and maintain professional relationships with people, carers, professionals and services, and share Bolton Council values.
Requirements
* Relevant qualification and professional registration as applicable.
* Full UK driving licence (essential car user allowance).
* Committed to safeguarding and welfare of vulnerable adults and young people; Enhanced DBS with safeguarding checks are required.
* This is a designated customer-facing role under the fluency duty and requires a specified level of spoken English as per the person specification.
