The Rough Sleeper Drug and Alcohol (RSDA) Team is an exciting and innovative team which provides a bespoke, assertive outreach, trauma informed addictions treatment model to those with substance misuse needs for those at risk of becoming homeless or are homeless.
The team is a Multi-Disciplinary Team of Consultant Psychiatry, Non-Medical Prescribers, Assertive Outreach workers, Recovery Coordinators, Neuropsychology and a Mental Health Practitioner.
The service is aimed at supporting people facing multiple needs and exclusion leading to them having difficulty in finding and sustaining suitable accommodation and increasing their risk of recurring homelessness.
As the post holder you have demonstrable experience of working in a health or social care setting and level 3 NVQ in health or equivalent experience. Knowledge of substance misuse and and its affects.
Main duties of the job
The post holder will provide community-based engagement, assessment and brief interventions to service users with substance misuse needs. The post holder will be part of the RSDA Team and will work alongside Local Authority staff, Rough Sleeper Initiative and Housing First Dual Diagnosis Practitioners, police and homeless outreach workers providing support to rough sleepers both through drop-in clinics at day centres or directly on the street whilst working alongside outreach workers.
To actively outreach to the homeless by connecting with them in their communities.
You willoffer advice, information, advocacy and support tohomelesssubstancemisusers, providinga range of interventions dependent on the needs of theindividual.
Within the bounds ofsafeguarding andconfidentiality youwilluse amultidisciplinary assessment approachwhen assessing the needs of the individual, taking care to build trustto increase their engagement with services.
Tocase managea small case load ofidentifiedservice userswith substance misuse problems, keeping them fully informed of the range of services,support available to them and the choices they have.Whilst ensuring thattheyare involved in developing realisticrecoveryplans and facilitating access toclinical,psychosocial, health, housinginterventionsand other services in their own communities.
