Job responsibilities

Patient Care and Management:
* Provide expert nursing care and management for patients with long-term conditions such as diabetes, asthma, COPD, and cardiovascular disease.
* Conduct thorough health assessments and develop personalised care plans in collaboration with patients and their families.
* Monitor and review patients' progress, adjusting care plans as necessary to ensure optimal management of their conditions.
Health Education and Promotion:
* Educate patients and their families on managing chronic conditions, including medication management, lifestyle modifications, and self-care techniques.
* Promote health and wellbeing through patient education sessions, workshops, and community outreach initiatives.
Collaboration and Teamwork:
* Work closely with the multidisciplinary team, including GPs, healthcare assistants, and other allied health professionals, to deliver integrated and coordinated care.
* Participate in team meetings and case discussions to ensure a holistic approach to patient care.
Quality Improvement and Professional Development:
* Engage in quality improvement initiatives to enhance patient outcomes and service delivery.

Qualifications and Experience:
* Registered Nurse with current NMC registration.
