FIELD CARE SUPERVISOR, Cambridge and surrounding areas

APT Care provide domiciliary care to elderly people within their own homes across Cambridge, with a designated CQC registered manager, office and community teams for this area.
APT are expanding their office team, looking for a new Care Supervisor based in the Cambridge office, supporting our teams every day. Areas of work within Cambridge CB 1 to CB 4
This role is based within the office with visits out in the community and provides our strongest link between services delivered and services monitored. The applicant must be highly organised, always professional and keen to achieve positive outcomes for people we are supporting in the local community.
* Completing home visit assessment following hospital discharge
* Professional meetings, emails and telephone communication
* Creating short term care plan documents to support hospital discharges
* Completing annual reviews in peoples properties, updating annual care plans
* Carrying out competency testing on community carers
* Signing off new carers recently inducted through our 'buddy supervisor' process
* Supporting managers with investigations and actions from concerns
* Delivering medication and manual handling training to new joiners or as annual refreshers
* Raising referrals to other professionals such as SALT, GP, Nurses, Safeguarding's & CQC
APT Care have a full time on call team in place for full out of hours coverage Monday-Sunday. This field care supervisor position requires a commitment of usually 1 weekend day a month as the nominated person to escalate to should an emergency arise that on call need to seek additional guidance on.
This position is supported by managers, coordinators and other field care supervisors working together at the APT Care Cambridge branch.
Full induction training provided along with an induction program, shadowing and 121 training with management.
