Job Description
We are currently seeking a Practice Nurse to join our supportive and well-organised team on a part-time basis. Located in Merseyside.
Days: 3-5 days - working pattern negotiable.
Hours: 20–30 hours per week
About the Role
As a Practice Nurse, you will be responsible for delivering high-quality care across various clinical areas, including chronic disease management and health promotion.
Duties may include:
* Running chronic disease clinics such as:
o Annual reviews for diabetes, asthma, and COPD
o Hypertension reviews
* Cervical smears, ECGs, urinalysis, phlebotomy, and wound care
* Suture/clip removal
* Contraception advice
* Vaccinations – routine, travel, seasonal (adults and children)
* New patient health checks and NHS health checks
* Providing health education and advice
* Occasionally undertaking home visits to carry out reviews and other nursing duties for housebound patients

* A modern, purpose-built facility with ample on-site parking
* A collaborative environment, working closely with local practices as part of a Primary Care Network (PCN)
* Rated Good in all areas by the CQC
* High QOF achievement
* Clinical system: EMIS Web
* Practice is open Monday to Friday, 08:00–18:30
Additional Responsibilities
* Maintain accurate and up-to-date clinical records
* Collaborate with the HCA and Phlebotomy team
* Diagnose and manage care for patients in the clinic or at home
* Promote opportunistic health education
* Manage stock control and infection prevention procedures
