Job Description We are currently seeking a Practice Nurse to join our supportive and well-organised team on a part-time basis. Located in Merseyside. Days: 3-5 days - working pattern negotiable. Hours: 20–30 hours per week About the Role As a Practice Nurse, you will be responsible for delivering high-quality care across various clinical areas, including chronic disease management and health promotion. Duties may include: Running chronic disease clinics such as: Annual reviews for diabetes, asthma, and COPD Hypertension reviews Cervical smears, ECGs, urinalysis, phlebotomy, and wound care Suture/clip removal Contraception advice Vaccinations – routine, travel, seasonal (adults and children) New patient health checks and NHS health checks Providing health education and advice Occasionally undertaking home visits to carry out reviews and other nursing duties for housebound patients Information about the surgery: A modern, purpose-built facility with ample on-site parking A collaborative environment, working closely with local practices as part of a Primary Care Network (PCN) Rated Good in all areas by the CQC High QOF achievement Clinical system: EMIS Web Practice is open Monday to Friday, 08:00–18:30 Additional Responsibilities Maintain accurate and up-to-date clinical records Collaborate with the HCA and Phlebotomy team Diagnose and manage care for patients in the clinic or at home Promote opportunistic health education Manage stock control and infection prevention procedures
