Job Overview
We are seeking a dedicated and compassionate Care Coordinator to join our team. The ideal candidate will play a crucial role in ensuring that our clients receive the highest quality of care and support. The Care Coordinator will also be responsible for maintaining accurate records and providing administrative support within the office environment.
Responsibilities
* Coordinate and manage care services for clients, ensuring their needs are met effectively.
* Liaise with healthcare providers, families, and other stakeholders to develop comprehensive care plans.
* Maintain accurate and up-to-date client records and documentation.
* Schedule appointments and follow-ups for clients as needed.
* Assist in the development of care policies and procedures to enhance service delivery.
* Provide administrative support within the office, including answering queries and managing correspondence.
* Monitor client progress and adjust care plans as necessary to ensure optimal outcomes.
* Participate in team meetings to discuss client cases and share insights for improved service delivery.
Experience
* Previous office experience is essential, demonstrating strong organisational skills and attention to detail.
* Experience in a healthcare or social services setting is advantageous but not mandatory.
* Excellent communication skills, both verbal and written, are required to interact effectively with clients and professionals alike.
* Ability to work independently as well as part of a team, demonstrating flexibility in a dynamic environment.
