Key Responsibilities
* Lead and coordinate Decision Support Tool (DST) assessments for NHS Continuing Healthcare (CHC) eligibility
* Manage complex, high-risk, and contested cases, including disputes and joint funding arrangements
* Work collaboratively with health partners, care providers, and local authority colleagues to develop appropriate care and support plans
* Undertake Section 117 reviews, ensuring statutory duties are met and care remains person-centred
* Contribute to service development and continuous improvement initiatives
* Provide professional supervision, mentoring, and guidance to a small team of social care staff
* Liaise effectively with internal and external stakeholders to deliver outcomes aligned with organisational priorities and values
Applicant Requirements
* Social Work qualification (Degree or equivalent)
* Registration with Social Work England
* Significant post-qualifying experience within Adult Social Care
* Strong working knowledge of CHC processes, including DST assessments and funding pathways
* Experience managing complex cases involving safeguarding, mental capacity, and multi-agency working
* Previous experience providing supervision or mentoring to staff (desirable)
* Sound knowledge of relevant legislation, including the Care Act 2014, Mental Capacity Act, and Section 117
* Excellent communication, decision-making, and organisational skills
* Ability to work autonomously while contributing effectively to a team environment
