An exciting opportunity has arisen to join the busy Children's Community Nursing Team as a Childrens Community Nurse to cover a 12-month maternity leave. The successful candidate will embrace the challenge of working across different environments to meet diverse service user needs. This is an excellent opportunity to work as an autonomous practitioner, gaining knowledge across an array of specialties and strengthening multi-professional working relationships. It is a role whereby excellent interpersonal skills, attention to detail, and professional curiosity are paramount.

As part of our recruitment process, you will also meet with our Youth Panel, which will last approximately 15 minutes. During this session, you will meet young people passionate about improving services at University Hospitals Plymouth. This initiative is part of an exciting pilot project at UHP, ensuring that the voices of young people are heard and contribute to decisions affecting them.

Responsibilities include providing a specialist community nursing service for children with acute and ongoing care needs within residential and educational settings, offering education, advice, and support through various stages of illness, and empowering families to manage their child's condition at home to avoid hospital admissions. The role is primarily for a generic children's community nurse, with opportunities to develop specialist knowledge and support within schools.

Preference will be given to internal Trust staff, Priority, and 'At Risk' status applicants, including NHS At Risk staff across Devon with relevant recent experience as per the Person Specification.

* Manage a caseload of children with ongoing medical/nursing needs, assessing, planning, implementing, and evaluating individual care programs.
* Accept referrals for children with specific care needs, in accordance with protocols.
* Liaise with paediatricians and multidisciplinary teams for discharge planning and transition from hospital to home.
* Visit children and families at home or other settings to deliver nursing care, monitor health, and provide support.
* Participate in multidisciplinary team meetings to implement complex care plans and ensure accurate documentation.
* Instruct parents/carers on clinical skills, such as feeding and device management.
* Perform nursing procedures, manage central lines, and administer IV drugs at home and in educational settings.
* Assess learning needs of children and parents, developing tailored education strategies.
* Identify social care needs, including safeguarding, and refer to Social Services as needed.
* Lead and coordinate multi-agency meetings to ensure coordinated care.
* Develop and implement evidence-based protocols across care settings.
* Maintain in-depth knowledge of conditions affecting children on caseload.
* Assess and organize equipment supplies, training families on their use.
* Facilitate smooth transfer from paediatric to adult services.
* Utilize problem-solving skills to prevent deterioration and hospital admissions.
* Stay updated with evidence-based practices and government guidelines.
* Admit children to hospital when appropriate, using professional judgment.
* Provide sensitive health education and promotion.
* Attend or run clinics, monitor ongoing conditions, and report to consultants as needed.
* Mentor nursing students, ensuring they meet competencies.
* Undertake non-medical prescribing if appropriate.
* Ensure a safe environment, maintaining equipment properly.
