Morley and District Primary Care Network have an exciting opportunity for a Health and Wellbeing Coach to join the team.
This is a new and important role that will help shape and form our local healthcare offer in Morley, Leeds.
The suitable candidate should be passionate about making a difference in primary care and enjoy working as part of a multi-disciplinary team across services.
The role will involve working across the 6 practices we support, so the candidate must be able to be flexible and adapt to different environments.

Main duties of the job
Health and wellbeing coaches are non-clinical, personalised care roles. They work with people with physical and/or mental health conditions, long-term conditions, and those at risk of developing them. They focus on improving health-related outcomes by working with people to set personalised goals. They support individuals to increase self-efficacy, motivation, and commitment to lifestyle changes.
The role involves assessing patients holistically, either face-to-face or by phone, and supporting them through motivational coaching and behaviour change to manage their long-term conditions.
You will coach and motivate patients through multiple sessions to identify needs, set goals, and support the implementation of personalised health and care plans. You will provide support to individuals, families, and carers to ensure active participation in healthcare, empowering them to take control of their health and wellbeing, live independently, and improve outcomes through joint care planning.

Job responsibilities
As a health and wellbeing coach, you will work with individuals with physical and/or mental health conditions, focusing on behaviour change and person-centred care. You will assist patients face-to-face or by phone, assessing their needs holistically and supporting them through coaching and behaviour change strategies.
You will support patients over an agreed period, building caseloads from referrals received from other professionals. Your approach will consider the whole person, encouraging proactive prevention, and supporting personal choice and positive risk-taking. You will ensure patients understand the accountability of their decisions, placing them at the centre of every conversation.
Person Specification
Qualifications
* NVQ Level 3, Advanced Level, or equivalent qualifications, or working towards them
* Training in motivational coaching and interviewing or equivalent experience
Experience
* Experience in community development, adult health and social care, learning support, or public health/health improvement (including unpaid work)
* Experience working in Primary Care
* Supporting people, families, and carers in related roles (including unpaid work)
* Supporting individuals with mental health issues in paid, unpaid, or informal capacities
* Experience working with the VCSE sector, volunteers, and community groups
