We are a high-achieving, single handed GP practice based within Stanhope Parade Health Centre, South Shields, serving a population of 2900 patients.
We are looking for dedicated and compassionate Practice Nurse to join our friendly team.
Main duties of the job
The ideal candidate should have practical and confident working knowledge of chronic disease management including diabetes and respiratory medicine, childhood immunisations, cervical screening, health promotion, contraception, adult vaccinations, wound care, ear care and travel health.
Excellent written and verbal communication skills.

We are looking for an experienced Practice Nurse to join our team.
You will be required to also work collaboratively with other members of the general practice team, including colleagues from the Primary Care Network and members of the wider community teams.
Job responsibilities
Thefollowing are the core responsibilities of the practice nurse. There may be onoccasion, a requirement to carry out other tasks; this will be dependent uponfactors such as workload and staffing levels:
a.Develop,implement and embed health promotion and well-being programmes
b.Implementand evaluate individual treatment plans for chronic disease patients
c.Identify,manage and support patients as risk of developing long-term conditions,preventing adverse effects on the patients health
d.Provideroutine nursing care to patients as required in accordance with clinical basedevidence, NICE and the NSF
e.Providewound care (ulcer /Doppler etc.) to patients
g.Requestpathology services as necessary
h.Processpathology results as required
i.Providechronic disease clinics, delivering patient care as necessary, referringpatients to secondary / specialist care as required
j.Maintainaccurate clinical records in conjunction with extant legislation
k.Ensureread codes are used effectively
l.Maintainchronic disease registers
m.Develop,implement and embed well woman clinics
n.Develop,implement and embed well man clinics
o.Chaperonepatients where necessary
p.AssistGPs with minor surgery when required
q.Prioritisehealth issues and intervene appropriately
r.Supportthe team in dealing with clinical emergencies
s.Recognise,assess and refer patients presenting with mental health needs
t.Implementvaccination programmes for adults and children
u.Supportpatients in the use of their prescribed medicines or over the counter medicines(within own scope of practice)
v.Liaisewith external services / agencies to ensure the patient is supportedappropriately (vulnerable patients etc.)
w.Delegateclinical responsibilities appropriately (ensuring safe practice and the task iswithin the scope of practice of the individual)
x.Supportthe clinical team with all safeguarding matters, in accordance with local andnational policies
y.Understandpractice and local policies for substance abuse and addictive behaviour,referring patients appropriately
Deliver opportunistic health promotion whereappropriate
Inaddition to the primary responsibilities, the practice nurse may be requestedto:
a.Actas the audit lead, effectively utilising the audit cycle
b.Monitorand ensure the safe storage, rotation and disposal of medicaments
c.Supportjunior members of the nursing team, providing guidance when necessary
d.Participatein local initiatives to enhance service delivery and patient care
e.Supportand participate in shared learning within the practice
f. Continually reviewclinical practices, responding to national policies and initiatives whereappropriate
Participate in the review of significant andnear-miss events applying a structured approach i.e. root cause analysis (RCA)
