Main duties of the job
We are looking for a motivated GP to undertakethe usual range of duties including surgery face to face consultations,telephone triage, video consultations, on-call, home visits (minimal) and theassociated administration including but not limited to reports, path labresults, documents and prescriptions in a timely fashion.
We offer chronic disease management clinics andhave an excellent nursing, pharmacy and healthcare team who lead on this,enhanced services including minor surgery and sexual health.
We have a successful dispensary withexperienced staff and led by our in-house Pharmacist providing support andadvice to patients and clinicians.
We are high QOF achievers, using EMIS Web, training practice, member of local GP federation, and welcomeany specialist interests.
Supported by a frailty team of occupational therapists,pharmacists, social prescribers, wellbeing coach, physiotherapists and safeguarding care co-ordinators.

Job responsibilities
* The practice timetable is designed to ensure the optimum delivery of our services to patients and other contractual obligations and these will vary over time.
* In accordance with this, and at the present time, the postholder will undertake a variety of duties including, but not limited to,
* Triage
* Telephone and face to face consultations and queries
* Visiting patients at home and in the community hospital
* Checking and signing repeat prescriptions and dealing with queries
* Handling results, correspondence and associated administrative tasks in a timely fashion (paper based, electronic etc).
* Participate and contribute to effective functioning of the practice team and its links to the wider communities
* Make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other healthcare workers within the organisation
* Assess the health care needs of patients with undifferentiated and undiagnosed problems
* Screen patients for disease risk factors and early signs of illness
* Develop care plans for health in consultation with patients and in line with current practice disease management protocols
* Provide counselling and health education
* Adhere to the clinical governance requirements of the practice
* Adhere to the information governance and information management requirements of the practice including the Quality and Outcomes Framework.
* Participate in and comply with the commissioning requirements of the practice
* Prescribe in accordance with the practice prescribing guidelines
* Attend clinical meetings/other meetings as required
* Undertake other appropriate duties agreed between the practice and the employee.
* In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.

* Fully qualified GP with GMC registration
* Medical Performers List registration
* Enhanced DBS
* Occupational Health Clearance
* Full driving licence
