We have an exciting opportunity for a highly motivated and autonomous HCPC Registered Band 6 Occupational Therapist to join our well established Home First Rehabilitation Service in the North of Staffordshire.

You will be part of the Allied Health Professions (AHP) service providing Occupational Therapy and will work in a variety of settings.

You will be responsible for delivering high quality rehabilitation to patients with a range of physical health conditions including MSK, Stroke, Parkinson's disease, Neurological conditions, Spinal Injuries, Cancer, Post Surgery, General Frailty, Amputees, plus a wide range of other conditions which provides you with a varied and exciting experience within our service.

We offer excellent support through CPD routes and building robust learning plans within the four pillars of AHP practice; offering opportunities to represent and promote our service locally, regionally, and nationally including UKSF & CSP conferences.

Due to this being a varied role you will be required to adequately travel across the geographical area to meet the demands of the role.

Main duties of the job

The role involves managing a caseload of adults in close liaison with the Home First Team. Many of these patients will have multiple complex health needs and will require you to work closely with uni/multi-disciplinary teams/agencies in addressing a range of care/support issues across the spectrum of conditions, preventing unnecessary hospital admissions and improving the health and wellbeing of our patients.

As part of this role you will be expected to:


* Be a specialist HCPC Registered Specialist Occupational Therapist and have a detailed professional and clinical knowledge.
* Be highly skilled in the assessment of complex/specialist care/support needs and in the development, implementation and evaluation of care/support packages.
* Have supervisory, training and leadership responsibilities where appropriate and will have some responsibility for team performance and service developments/improvements.
* Demonstrate initiative and be creative in finding solutions to problem solving.
* Work with the team leader undertaking delegated tasks in team management.
* Promoting independence/self-management and maintaining safety.
* Increased functional ability.
* Improvement in health and wellbeing including mental health.

Qualifications And Training

Essential criteria

* Degree in Occupational Therapy or equivalent.
* Registration with HCPC/NMC
* Fieldwork educators accreditation
* Hold a valid driving licence and access to a vehicle

Desirable criteria

* Modules at Masters level
* Evidence of further clinical study
* APPLE accreditation
* Leadership experience/qualification
* Membership of BAOT / COT

Experience

Essential criteria

* Post qualification courses in working with older people and people with physical disabilities.
* Attendance at relevant clinical networks or Special Interest Groups
* Up to date field work/pre-registration student training.

Desirable criteria

* Experience in Experience of using standardised assessments and outcome measures
* Experience of negotiation and conflict management in clinical situations.

Skills, Knowledge And Experience

Essential criteria

* Relevant experience to the role including assessment and interventions for a wide range of disabilities.
* Ability to manage time and caseload effectively.
* Supporting service users, families and carers in dealing with the impact of disability and life limiting conditions
* Working in a multidisciplinary team
* Flexibility with working hours to enable 7 day working Evidence of specialist clinical knowledge relevant to the post.
* Ability to demonstrate an in-depth knowledge of a variety of medical conditions and disease processes, equipment and minor home adaptations.
* Understanding of Information Governance, Data Protection, HCPC /COT standards and Safeguarding Vulnerable Adults.
* Working across hospitals and community settings and have the ability to travel within a geographical area to meet the requirements of the post.
* Assess individual's psychological, social and emotional needs for rehabilitation
* Prioritise treatment and care for individuals according to their health status and needs
* Discharge and transfer individuals where appropriate
* Support individuals to manage their condition/disability
* Awareness of relevant NSF's and NICE guidance

Desirable criteria

* Knowledge of relevant NSF's and NICE guidance. Understanding of wider NHS agenda and its application to the local context.
* Continuing healthcare assessments
* An understanding of team dynamics
