Primary Care Senior Staff Nurse - HMP Send. Central and North West London NHS Foundation Trust is a Top Employer Award Winner and works in partnership to provide responsive and dependable primary care, mental health, intellectual disability and substance misuse services to the diverse communities we serve. This role is to recruit a highly motivated, innovative and enthusiastic Primary Care Senior Staff Nurse to join our Health & Justice team at HMP Send. Our Prison Health Care Team delivers healthcare in secure environments with a 7 day week model to ensure high standards at all times. Our motto is Caring NOT Judging, and we work in partnership with the criminal justice system and other agencies to provide high-quality primary care and mental health services to those who may have offended and those at risk of offending, aiming to improve health and social inclusion.

Main Responsibilities
* Plan, deliver and evaluate clinically appropriate interventions using evidence-based guidance and practice.
* Set up processes and protocols for effective assessment, planning, implementation and evaluation of care for patients with physical health, mental health and substance misuse needs.
* Safeguard vulnerable patients in line with CNWL Trust safeguarding policies.
* Apply local risk assessment and management; make appropriate referrals for long-term or complex care management.
* Coordinate multi-disciplinary clinical practice to provide focused and integrated care programs.
* Use highly skilled interventions to de-escalate aggressive patient behaviours and follow appropriate processes for handling such situations.
* Work collaboratively with prison, education staff and other disciplines to provide integrated care and maintain open communication with stakeholders.
* Operate within the sphere of competency and communicate training needs to the team.
* Participate in local and trust audits and clinical governance; utilise research skills for audit, service development and research within the multi-disciplinary area.
* Liaise with other professional services, including hospital staff, GPs and caseworkers, sharing information appropriately.
* Maintain high standards of clinical record keeping, including electronic data entry; ensure professional activity reflects diversity considerations in care delivery.
* Attend and contribute to prison meetings as required and communicate actions to the responsible person.
* Develop care pathways, collect collateral information, and make appropriate referrals into the community when patients approach discharge or transfer.
