Job Overview PLEASE NOTE THAT WE WILL ONLY ACCEPT APPLICATIONS FROM STAFF CURRENTLY EMPLOYED BY BETSI CADWALADR UNIVERSITY HEALTH BOARD An exciting opportunity has arisen for a number of Associate Practitioners within East Area. The posts are a new and innovative role based within the Rehabilitation Unit. The post holder will work as part of a designated Team delivering evidence based high quality clinical care that focuses on the direct needs of the patient including those individuals with chronic conditions. The post holder will work under the direction and support of a Registered Nurse by encompassing planning, organising and delivering clinical care. Training and support will be provided to achieve the required clinical skills and knowledge. The hours of work will be 37.5 per week. Flexibility will be required. Main duties of the job Assist in identifying health needs of patients within scope of competence. encompassing planning, organising and delivering clinical care as agreed with the Supervising Practitioner/Registered Nurse, in line with clinical governance and within agreed professional standards and guidelines. Work flexibly as part of the nursing team providing health promotion, and health education to adults and their families including smoking cessation, diet, exercise and healthy lifestyle information empowering the patient towards self management. Work in partnership with other local agencies such as social services and the voluntary sector to mobilise resources Undertake care of wounds that require simple or more advanced dressings, document care given and report back findings to relevant nurse. Undertake continence re-assessments and management with the competency to carry out bladder scanning and the catheterisation procedure assessed as necessary in consultation with the Registered Nurse. To implement the use of evidence based assessment tools as required for example the Falls Risk Assessment Tool for those individuals at risk of a fall, reporting findings to the Registered Nurse and providing appropriate interventions in terms of falls prevention and management. The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply. Person specification Qualifications Essential criteria QCF Diploma in health and social care level 4 Five GCSEs at grades A – C or equivalent level of qualification, e.g. NVQ level 3 in Health and Social Care related subject Be able to demonstrate literacy and numeracy to key skills Level 2 (equivalent to GCSE grade A - C in English and Maths) Experience Essential criteria Previous relevant experience in a healthcare environment working in an indirect supervised role Desirable criteria Experience of working at a higher level as a healthcare support worker. Experience of working with patient complex needs resulting from their chronic condition(s) Skills Essential criteria Evidence of good organisational skills Excellent interpersonal skills The ability to care with compassion and understanding. Effective problem solver Ability to negotiate and communicate with patients and all members of the Multidisciplinary Team Evidence of IT skills e.g. e-mail, MS Word, Excel and Office Demonstrate good verbal communication and written skills. Knowledge of local policies/procedures and guidelines including those specific to the community Knowledge of the Fundamentals of Care Recognise the need for lifelong learning Able to relate theory to practice. Knowledge, skills and competences to identify problems, analyse and compare appropriate information before reaching a judgement, making decisions and responding accordingly within own boundaries An insight into how to evaluate own strengths and development needs, seeking advice where appropriate Knowledge, skills and competences for and contribute to the assessment and planning process for patient care needs.
