Overview
Provide a Specialist Clinical Pharmacy Service to adult oncology and haematology ward and the acute emergency assessment unit.
Lead, develop and provide a specialist clinical pharmacy service to the adult oncology and haematology ward and the acute emergency assessment unit. Collaborate with oncology and haematology clinicians to implement and promote safe, high quality, evidence-based prescribing and safe management of medicines. Communicate highly complex information relating to medicines with clinicians, nurses, independent prescribers, carers and patients. Act as a highly specialised clinical pharmacist and provide a patient-focused clinical pharmacy service on the adult oncology and haematology ward and the acute emergency assessment unit, including proactive and reactive participation in ward rounds, input into medicines management review, counselling, acting as a clinical pharmacy role model, and being accountable for own professional actions. Develop and evaluate the delivery of a responsive, proactive, patient-focused Clinical Pharmacy Service for patients receiving treatment for cancer on the ward, in the emergency assessment unit, and in associated outpatient and day-case settings.
Responsibilities
* Specialist Clinical Pharmacy Service to Adult Clinical Oncology and Haematology – inpatient, emergency assessment unit, outpatient and day-case units for haematology and oncology.
* Deliver a high level Specialist Clinical Pharmacist Service to Adult Clinical Haematology and Oncology, including the provision of a daily Clinical Pharmacist Service to inpatients on the ward, outpatient clinics and day-case chemotherapy.
* Use specialist knowledge and expertise to make complex judgments and provide expert clinical advice, evaluating evidence and advising in situations where evidence is lacking in Haematology and Oncology.
* Work as a prescriber in haematology clinics.
* Participate in daily ward and board rounds on the ward, attend MDTs, and be available to advise and train junior staff on the ward and emergency assessment bay.
* Provide general and specialist advice and information relating to medicines to all levels of health professionals, senior managers and patients within the specialised area to enhance the quality of patient care.
* Provide pharmaceutical support and information necessary to manage medicines safely and cost-effectively.
* Plan appropriate audits and practice research and implement appropriate service changes to Adult Haematology and Oncology.
* Lead the development and implementation of medicine-related guidelines, policies and procedures.
* Make and communicate appropriate clinical interventions as part of the service and discuss with Pharmacists in the Cancer Services Team.
* Support the Principal Pharmacist to investigate incidents and complaints relating to medicines usage within the specialist area and discuss with Pharmacists in the Cancer Services Team.
* Adhere to local and national guidance and policies.
* Supervise, check and, if necessary, undertake the supply of medicines.
* Optimise medicines through prescribing role operating within areas of competence (where appropriately qualified and authorised via the trust NMP register).
* Maintain up-to-date clinical expert knowledge in Clinical Pharmacy and pharmacy practice to ensure a broad base of pharmaceutical knowledge and advanced clinical reasoning and judgment.
* Fulfil the requirements of the Code of Ethics and Standards of Good Professional Practice.
* Implement the Duty of Candour.
