Job Summary:

SCOPE AND RANGE OF THE ROLE
4. Using specialist practitioner nursing skills to deliverthe highest standards of nursing care in accordance with evidence-based practice and nationalpractice protocols
5. To manage and develop a clinical acute and chronic caseload, dealing with presenting patients needs in a primary care setting
6. To assist with the development of the nursing servicesoffered by the practice
7. To provide clinical leadership within the nursing team
8. Review medication for therapeutic effectiveness,appropriate to patients needs and in accordance with evidence-based practice and national practice protocols
MAIN TASKS, DUTIES AND RESPONSIBILITIES
Clinical Role
9. Make professionally autonomous decisions for which he/sheis accountable and provide safe, evidence based, cost effective, individualised patient care
10. Monitors, Interprets and evaluates patient test results inagreement with the patient and their GP, ensure appropriate course of treatment is initiated, orrefer on if required
11. Delivers care and support planning and anticipatory care
12. Make appropriate referrals to primary, community andsecondary health care teams
13. Delivers person centred care approaches includingsupporting self management and behaviour change, practising motivational interviewing, deliveringcompassionate care and adopting personal outcomes asset- and strength based approaches andco-production
14. Understands public health, including health inequalities,epidemiology and prevention
15. Utilises a range of skills elated to e-health andtechnology to enable care and support self management
16. Initiate appropriate investigations for patients suspectedto have chronic disease diabetes, COPD, asthma, CHD, CVD, Heart Failure, Hypertension: referringto other clinicians as appropriate
17. Assist practice to develop and deliver specialist primarynurse led services including chronic diseasemanagement.
18. Assesses and manages long term conditions: has in-depthexpertise in long-term condition assessment, including comprehensive clinical examination toaddress highly complex health needs and physical and mental health assessment.
19. Formulates an action plan for the treatment of the patientwith long term conditions, synthesising clinical information based on the patients presentation,history, clinical assessment and findings from relevant investigations, using appropriate evidence-basedpractice
20. Manages programmes of care for patients with chronicdiseases by planning, providing and evaluating care under agreed guidelines and patient groupdirections. Maintain disease registers in liaison with administrative staff. At annual review willassess patients understanding and ability to self-manage
21. Initiate and carry out programmes of health screening asagreed with the practice clinical team: promote health and wellbeing, giving general education andadvice on weight, diet and healthy eating, activity and exercise, smoking cessation, alcoholconsumption and chronic disease management
22. Implements non-pharmacological relatedinterventions/therapies dependent on situation and technical requirements of care
23. Uses new knowledge in innovative ways and takesresponsibility for developing and changing practice in complex and sometimes unpredictable environments
24. Undertake regular medication reviews for groups ofpatients deemed suitable by the clinical team
25. Provide up to date contraceptive and sexual health advice,
26. Administer child and adult immunisations and vaccinationsin accordance with General Practice contract and following national and local programmes.Competent and up to date with anaphylaxis and resuscitation techniques.
27. Up to date and followguidelines regarding cold chain ensuring safe storage, rotation, and disposal of vaccine and drug, andthat adequate stock levels are maintained
28. Competent in procedures including venepuncture, BPmanagement, urinalysis, blood glucose testing, peak flow technique, spirometry, pregnancy testing,cervical screening, wound management, taking swabs
29. Provide support and chaperoning to GPs when requiredincluding in minor surgery clinics and other procedures
30. Enables, supports and encourages individuals, families,carers and groups to address issues which affect their health and social well being
31. Contributes to the practice achieving GMS contractobligations
32. Maintain accurate records following NMC standards ofrecord keeping
33. Ensure all data protection requirements are met whengathering, recording and storing patient data
34. Liaise and maintain good working relationships with allmembers of the practice team. Work within the MDT within the practice and across the widerhealth system

45. Participates in teaching of other members ofstaff within the surgery team is involved.

Lead Nurse
As the surgery's appointed Lead Nurse, assumeslead responsibility for:
47. Vaccinations and immunisations (includingseasonal flu vaccinations) to include but not limited to:
48. Review and dissemination of vaccination andimmunisation based education and updates to the clinical team;
49. Coordination of PSDs; and overseeing compliance with national requirementsand service specifications, to include overseeing recallssupported by the nursing and administration team, in particular the ClinicalPerformance Manager.
50. Infection control to include, but not limitedto:
51. Overseeing and updating the surgerys infectioncontrol policy, procedures and protocols;
52. Overseeing and arranging in house infectioncontrol training; and
53. Undertaking required infection control audits,producing associated reports and overseeing implementation of any requiredactions.
54. INR to include, but not limited to:
55. Overseeing and updating the surgerys INRpolicy, procedures and
56. protocols;
57. Proving in-house INR support and training; and
58. Overseeing appropriate operation and maintenanceof INR Star.
59. Cold chain compliance to include, but notlimited to:

Nurse Manager
Assume day-to-day responsibility for the operational(non-contractual) management of the surgery's nursing team to include, but notlimited to:
62. Attending the surgery's daily huddle and reporting onstaffing and clinics from a nursing perspective;
63. Preparing weekly nursing rotas to ensure good access tonursing clinics maintained;
64. Attending weekly leadership meetings and reporting onstaffing, clinic and
65. other associated matters from a nursing perspective
66. Overseeing, managing and recording nursing team leaverequests and absences, to include requesting cover in the event ofnursing team absences; and
67. Subject to training and supported by the GPs and PracticeManager, undertake annual appraisals with the nursing team.
