We are a dedicated domiciliary care organisation seeking a Care Coordinator with senior-level experience in care delivery and service management. This is a dynamic role that blends strategic oversight with hands-on support, ensuring our clients receive the highest standard of care.
Key Responsibilities:
* Coordinate and manage care services across the community
* Conduct assessments of need and develop tailored care plans
* Lead care reviews and ensure plans remain responsive and effective
* Manage medication processes and complete regular audits
* Carry out thorough risk assessments in varied and sometimes complex situations
* Support the delivery of care during unsociable hours when required
* Oversee and participate in the out-of-hours on-call rota
* Respond to emergencies and ensure continuity of care during shift cover gaps
What We're Looking For:
* Proven experience in senior care delivery and service coordination
* Strong understanding of medication management and auditing practices
* Confident in conducting risk assessments and navigating challenging scenarios
* Experience managing out-of-hours services and emergency response
* A Level 3 qualification in Health and Social Care (or equivalent)
* A full UK driving licence and access to a car for work purposes
* A flexible approach to working hours, including occasional field-based care
Working Pattern:
* 32 hours per week
* Office hours: 8:30am–4:30pm
* Flexibility required for occasional field visits and unsociable hours
