About the Team
We are looking to recruit to an already existing team with 2 fixed term contract posts. One will be a six-month contract and the other a 12-month contract. To help us fill an exciting role within the Hospital to Home team.Both roles will be working out of our Leamington HQ hub.
Please be aware there is a consultation that will be under way whilst we carry out this recruitment, start and finish times will be part of that consultation, these will be discussed at interview and confirmed. Both roles will be working from our Leamington Headquarters base.
This is an excellent opportunity to work within a dynamic team contributing to and supporting all aspects of getting people home from hospital, in a safe manner, whilst settling them in their home to promote a healthy recovery after being in hospital. This supporting the NHS, Public Health and our Fire Service strategies in making communities safer within Warwickshire.
About the Role
We are looking for a highly motivated individual who will assist in the delivery of Hospital to Home and Prevention activities; working collaboratively to drive change internally and further strengthen our Health partnerships as part of the PPR Strategy.
As part of the role you must be committed to Safeguarding Children and Vulnerable Adults whilst carrying out your work. You will also be required to undergo an enhanced DBS check. We would also expect you to uphold the principles of the Core Code of Ethics.
