Job Title: Advanced Practitioner - Adults Safeguarding - Older Persons Team - Hertfordshire Location: Hertfordshire (Hybrid) Pay Rate: £34 per hour About the Role: A council is looking for Advanced Practitioners to join their older persons team. What will your responsibilities be? In this role, you will be responsible for delivering high-quality statutory social care, including safeguarding, assessment, supervision, and care management functions, as well as developing innovative community solutions. Advanced Practitioners are pivotal in developing links with the local community through multi-agency work with partner organisations, providers, professionals, individuals, families, and carers. You will ensure that all statutory health and social care responsibilities are met and delivered to a high standard. Your role will be essential in maintaining the well-being and safety of the community. In addition, you also need to actively be in any of the specialist roles, such as Moving and Handling Practitioner, Practice Educator, Approved Mental Health Practitioner, or Best Interest Assessor, or agree to undertake a specialist role within the first year of the role. Essential requirements: Strong written and verbal communication Minimum of 2 years of post-qualification experience.
