An exciting opportunity has arisen for arole as aClinical Nurse Specialist within the Specialist Palliative Care team at North Tees and Hartlepool NHS Foundation Trust.We are inviting applications from highly motivated registered nurses. You will have advanced communication and interpersonal skills and are educated to degree level. The applicant should have post registration experience working with palliative and end of life patients. A key part of the CNS role is to assess and facilitate appropriate transfers to local hospices working closely with the MDT and other stakeholders to ensure timely, safe and efficient transfers of care. You will be responsible for the implementation of timely and person-centred assessments to determine appropriate admission into the hospice setting for patients who require complex symptom management and/or end of life care. You will also contribute to avoid unnecessary hospital admissions/expedite discharges to meet the patient's preferred place of care or death.


Main duties of the job

Band 6 Clinical Nurse Specialist (CNS) holding a supervised clinical caseload, coordinating holistic care and providing on-going support for patients, their relatives and carers. The successful applicant will work alongside the Clinical Nurse Specialists, complete on going assessment, review, planning and evaluation of care requiring clinical expertise within the area of Specialist Palliative Care escalating any concerns to senior members of the team as required. Communication will be effective, empathetic and reassuring when communicating highly sensitive information and the CNS will act as patient advocate, and palliative care resource within Multidisciplinary meetings participating in the discussion on clinical management of patients on the SPC caseload

You will ensure continuous development of clinical skills in relation to symptom management for patients on the specialist palliative care caseload, ensuring timely escalation to senior clinical nurse specialists or SPC consultants when needs fall outside of the associates scope of practice or competency.Proactively monitor patients on the caseload recognising the early signs of the deteriorating patient or acute illness based on an understanding of the disease process.

CNS will conduct assessment and care planning to meet patients' future care needs to enable patient's wishes and preferences to be met and shared including Emergency Health Care Planning, Preferred Place of Care and DNACPR.

Job responsibilities

To act as Band 6 Clinical Nurse Specialist (CNS) for a supervised clinical caseload coordinating holistic care and providing on-going support for patients, their relatives and carers. Working alongside the team of Clinical Nurse Specialists, complete on going assessment, review, planning and evaluation of care requiring clinical expertise within the area of Specialist Palliative Care escalating any concerns to senior members of the team as required.

You will identify in-patients whose preferred place of care or death maybe home hospice or otherresidential setting. The successful applicant will assess patients who require symptom control orrequire end of life care and try and meet the patients PPC/D in a timely manner.You will work alongside the acute Specialist CNS team in hospital and community.

You will act as the patient advocate, palliative care resource within the multidisciplinary meetings,participating in the discussion on clinical management of patients on the SPC caseload,escalating concerns for discussion when appropriate.

Review patient medications in accordance with current Palliative Care guidance referring on forCNS/Consultant review when required to ensure patients receive maximum therapeutic support.

Proactively monitor patients on the caseload recognising the early signs of the deterioratingpatient or acute illness based on an understanding of the disease process.


Person Specification


Qualification

* Degree is related subject
* Qualification in Non medical prescribing or willing to work towards
* Qualification in Physical Assessment skills or willing to work towards


Experience

* Relevant post reg experience in Palliative care or cancer care
* Minimum of three years experience of working at Band 5
* Experience of teaching
* experience of working in community setting
* Ability to work autonomously and as part of a team
* Organisation skills, able to prioritise work load
* Advance communication qualification eg: Sage and Thyme, Psychology Level 2 etc


Disposition

* Ability to deal with complex and difficult emotional situations

£37,338 to £44,962 a yearpro rata per annum
