Job Summary: St John's Medical Centre has an exciting opportunity for a Practice Nurse to join our enthusiastic and supportive Practice Team. The successful candidate must be organised, motivated and caring to ensure the highest levels of care are delivered to our patient population through patient assessments, diagnosis and clinical management.

St John's Medical Centre is a large, fast paced GP surgery located in Grantham, offering excellent transport links with easy access to the A1 and the mainline for London. We currently have circa 13,000 patients registered and retained a 'Good' rating in our latest CQC Regulatory Review. The role is for 30 hours a week and includes working: Monday, Tuesday, Wednesday and Friday. Pay is dependent on experience.


Main duties of the job

* To be responsible for the delivery of practice nursing services, working as part of the practice's multidisciplinary team who provide care within their scope of practice to the entitled patient population.
* To be involved in health promotion, chronic disease management, health prevention, well woman and well man clinics, and supporting the management team in reviewing clinical policy and procedure.
* Carrying out home visits to patients with urgent clinical needs identified, including those escalated by EMAS or a member of the Neighbourhood Team.
* Liaise with primary, secondary and specialist care services as required.
* Work with the Neighbourhood Team to identify patients at risk of loss of independence or admission to hospital due to inadequate social support.
* Act as a point of contact between GP, Neighbourhood Team, patients and their carers; ensure care plans are communicated to GPs and other team members.
* Maintain accurate and up-to-date records of patients consultations using relevant software (e.g. SystmOne).
* Complete relevant health checks in line with patient needs and the ability to work alone when undertaking home visits/work in the community.
* Develop, implement and embed health promotion and well-being programmes; implement and evaluate individual treatment plans for chronic disease patients.
* Identify, manage and support patients at risk of developing long-term conditions; provide routine nursing care in line with clinical-based evidence, NICE and the NSF.
* Provide wound care, travel medicine services, request and process pathology services as necessary, and provide chronic disease clinics.
* Maintain accurate clinical records in line with extant legislation and ensure read codes are used effectively; maintain chronic disease registers.
* Develop, implement and embed well woman and well man clinics; chaperone patients where necessary; assist GPs with minor surgery when required.
* Prioritise health issues, intervene appropriately and support the team in dealing with clinical emergencies; recognise, assess and refer patients with mental health needs; implement vaccination programmes for adults and children.
* Support patients in the use of prescribed medicines or over-the-counter medicines (within own scope of practice); liaise with external services to ensure patients are supported appropriately.
* Delegate clinical responsibilities safely within the scope of practice and support safeguarding matters in accordance with local and national policies.
* Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately.

St John's Medical Centre is a large, well-established Practice with circa 13,000 patients registered. We are a Training Practice and regularly welcome GP Registrars, Nursing Placements and Work experience from local colleges/schools. In our last CQC Inspection we retained our 'Good' rating.
