There is an exciting opportunity for a Senior Occupational Therapy position within Tower Hamlets Reablement services.

The successful candidate will be required to manage a case load as well as undertake management responsibilities including, supervision, waiting list management and allocating cases appropriately. The role would also include planning and participating in the development of OT service in Tower Hamlets

Main duties of the job

* To provide specialised client-centered occupational therapy interventions to clients (aged 18 or over) who are resident in the community. This includes clients who may present with complex physical, cognitive, psycho-social and housing needs as a result of a wide range of medical conditions.
* To be an influential member of the multi-disciplinary team, providing expert, integrated, client-centred, goal- orientated rehabilitation for clients
* To work as an autonomous practitioner in managing a defined caseload of complex cases, in a community setting. To evidence high levels of problem-solving and clinical reasoning skills in line with evidence-based/client-centred principles. To formulate a diagnosis and treatment plan designed to reduce the impact of disability from a comprehensive assessment of the client's needs. To assess any on going care and support needs.
* To take a lead role in the supervision, teaching and appraisal of junior occupational therapists, rehabilitation support workers, students and other members of the multidisciplinary team.
* To work collaboratively with members of the multi-disciplinary team in the planning, development and evaluation of the Reablement Team, taking a lead for designated projects. To continually engage with and develop the provision of occupational therapy within community services.

Job responsibilities

This role is for a Band 7 Occupational Therapist, who will be based an integrated Reablement Team based in Adult Social Care.

The Band 7 Occupational Therapist will provide a high-quality, specialist service to adults and older people with a wide range of disabilities and complex needs, including chronic illness and long-term conditions. This community-based role sits within an integrated multidisciplinary Reablement Team and focuses on promoting independence, preventing hospital admissions, and supporting timely discharges through client-centred, goal-oriented interventions. The post holder will carry out comprehensive assessments, including Care Act Assessments and deliver occupational therapy interventions in clients homes and local communities, working collaboratively with service users, carers, and professionals to develop personalised care plans that support independent living.

In addition to clinical responsibilities, the post holder will provide leadership, supervision, and support to junior occupational therapists and Reablement Officers. The role also involves working closely with the OT Clinical Specialist to contribute to service development, training programmes, and clinical pathways, as well as participating in audits, research, and quality improvement initiatives.

As part of Role, the occupational therapist will collaborate with physiotherapists, nurses, social workers, GPs, and other professionals to deliver coordinated care that reduces unplanned hospital admissions and facilitates safe, supported discharges. The post holder will be expected to demonstrate strong clinical reasoning, leadership, and communication skills, and to act as a role model for best practice within the team.

Qualifications and Experience

* Occupational Therapy Degree
* Registered with HCPC
* Maintenance of CPD portfolio
* At least 3 years' post registration clinical experience in OT role, including community working with older adults with a physical health need
* Experience in supervising, managing and training junior staff
* Ability to identify and assess the potential risks involved in work activities and processes for self and others
* Experience in training, supervising and managing others including HR management


Skills and Abilities

* Ability to communicate with wide range of professionals
* Ability to establish a therapeutic relationship with patients and their families/carers
* Ability to undertake all aspects of the OT process
* Evidence of advanced clinical reasoning skills
* Ability to apply safe manual handling techniques with patients with complex needs and teach others
* Ability to manage individual and team workload independently
* Ability to support junior team members in all aspects of their role and ensure team functions at highest level
* Understanding of the legal responsibilities of the profession, including social care legislative frameworks
* Knowledge of current and emerging Social Care and NHS strategy and policy
* Ability to prioritise, organise and delegate
* Experience in training and educating other professionals in clinical practice
* Ability to self-reflect, carry out tasks of own job and identify what s/he needs to learn to able to do current job better
* Experience of working in a community setting


Knowledge

* Knowledge of theory and practice of OT with patients with physical, cognitive and psychosocial needs
* Knowledge of legislation affecting the work of OTs
* Knowledge of the roles of other disciplines and agencies involved in patient care
* Knowledge of the principles of clinical governance and their application
* Knowledge of standardised assessments appropriate to this patient group
* Knowledge of the Care Act
