Overview:

This hybrid role combines in-person home visits with remote coaching—no nights, weekends, or on-call duties.
Key Responsibilities:
* Conduct home visits to assess safety, monitor health changes, and provide clinical coaching (including medication reviews).
* Deliver ongoing coaching via our digital app, offering resource referrals and best practice guidance.
* Build trusting relationships with caregivers to reduce burnout and encourage positive changes.
* Maintain accurate, timely documentation in compliance with regulations.
* Assist with additional projects and duties as needed.
Requirements:
Education:
* ActiveRegistered Nurse (RN) Licensein the state of employment.
Experiences:
* 2+ yearsof recent direct care experience with elders/adults with disabilities.
* Background incase management, care planning, and assessments(eldercare, disabilities, or complex medical needs preferred).
Other:
* Must be able to travel within the assigned area (valid driver’s license and auto insurance required).
* Proficient inMS Office, case management systems, and web-based applications.
* Bilingual skills are a plus(may impact compensation).
Compensation and Benefits:
* Salary:$90,000–$100,000 per year
* Benefits:
