About the Role:
Trafford are currently recruiting an experienced Social Worker to join their Integrated Community Service within the Central Neighborhood Team, based at Sale Waterside. This is a fantastic opportunity to work within a supportive, multidisciplinary team delivering services to adults with a wide range of needs.
Key Responsibilities:
The successful candidate will:
* Undertaking Care Act assessments and developing outcome-focused support plans.
* Managing a caseload of adults with complex needs.
* Applying a strengths-based approach to assessments and interventions.
* Securing appropriate levels of funding in line with Reshaping Trafford Policy where required.
* Responding to Adult Safeguarding concerns and undertaking safeguarding enquiries in line with the Care Act and local procedures.
* Working collaboratively with health professionals and partner agencies to deliver coordinated support.
* Contributing to the team duty rota, responding to urgent situations and individuals in crisis.
