Current Opportunity: Care Manager We are seeking a compassionate Care Manager who can work full-time across our extra care scheme. Main Responsibilities: To promote a caring environment for clients through high standards of professional practice which are conducive to the physical, emotional, social, intellectual, and spiritual needs of the clients. To ensure that each client receives care appropriate to their individual needs. To implement the requirements of the Health and Social Care Act 2008. Management of client care packages. Management of Care Workers. Liaison with professional network, client network, and other members of the community where relevant. Main duties of the job: Managing client enquiries and providing all relevant information and leaflets to prospective new clients and visiting them in their home for a discussion. Attending client initial interviews, assessing client needs, and identifying any risks. Deciding whether or not the agency is able to meet the personal care needs of any prospective client and negotiating an appropriate fee with the purchasing authority, or the client or clients family if not in receipt of local authority assistance. Ensuring clients are provided with all necessary information and documentation e.g. client home folder, report sheets, etc. Completing and ensuring that client paperwork is in place and up to date (care plans, risk assessments, medication forms, reviews, etc). Managing and monitoring care packages. Following up on all issues reported and ensuring they are recorded on HCR and forwarded to the relevant person. Reporting all activities, concerns, and communication of all relevant information to representatives, professional network, etc. Ensuring that all complaints are dealt with in line with the complaints procedure. Ensuring that the emotional, spiritual, physical, medical, and material needs of the service users are recognised, assessed, and met. Staff matters: Ensuring that effective induction, supervision, and assessment of staff are carried out and that training needs are identified and met. Supporting Care Workers during their induction week, working alongside all Care Workers to ensure they maintain a good standard. Ensuring Care Workers hold relevant information on clients conditions. Assessing clients homes with regard to health and safety and infection control issues and providing advice to staff regarding safe and healthy working practices in the field. Person Specification Essential Criteria: The following qualities are considered essential for the post of a Care Manager: At least two years of experience in a management capacity within the previous five years.
