Who You Are You are a professionally qualified Social Worker with at least two years of post-qualification experience. You are committed to strengths-based practice and have skills in supporting adults with care and support needs. You are capable of managing a varied and complex caseload autonomously while mentoring other Adult Care practitioners. You are familiar with working in a multi-agency environment and capable of integrating your practice across the health and care sector. What the Job Involves As a Social Worker Level 2, you will provide statutory professional social work under the framework of the Care Act 2014. Your responsibilities will include assessment, care and support planning, and reviewing care packages for adults with physical disabilities and frailty. You will collaborate with individuals, families, and agencies to construct value-for-money care packages, undertake safeguarding enquiries, and work with specialist safeguarding colleagues. You will also deputize for Lead Practitioners when needed, supervise Community Care Officers and students, and ensure compliance with Lincolnshire County Council policies.
