Mental Health Practitioner - Home Based Treatment Team
An opportunity has arisen for a Band 6 Registered Mental Health/Learning Disability Nurse, Social Worker, or Occupational Therapist to join the North Home-Based Treatment Team to cover maternity leave. This dynamic and driven multi-disciplinary team offers an alternative to psychiatric hospital admission, supporting service users in acute mental health crises in their homes, as well as supporting loved ones and carers during this period.
The team is passionate about providing person-centred care, tailoring care plans to meet individual needs, and delivering a range of psychosocial interventions. The team is required to thoroughly risk assess situations and has access to supportive leadership for complex cases.
Staff wellbeing and development are paramount, with full supernumerary induction provided for the first 4 weeks. The post holder will be supported in their professional development, with regular supervision and opportunities to participate in service development projects. The team operates a 24/7 service, so shift work will be required.
Main duties of the job
* Collaborative care planning with service users and their carers.
* Risk assessment and recording risks according to trust policy.
* Shift co-ordination when allocated.
* Participation in multidisciplinary team meetings and care planning.
* Maintaining knowledge of evidence-based home treatment interventions and delivering these interventions.
* Gatekeeping assessments to ensure treatment in the least restrictive environment.

Working hours: Saturday (midnight to midnight), any weekday after 8pm and before 6am, and all time on Sundays and Public Holidays.

Salary: £37,338 to £44,962 per annum (pro rata).
