Cardiac Surgical Liaison & Rehabilitation Nurse Specialist
An exciting opportunity to work within the Cardiac Specialist Nursing Team at The Royal Cornwall Hospitals NHS Trust.
If you are looking for a part‑time role, this position is 26.25 hours per week: 2 weeks of 3 days and 2 weeks of 4 days each month, based at the Royal Cornwall Hospital, Truro. The role requires travel to exercise groups in central and west Cornwall.
Responsibilities
* Cardiac Rehabilitation (CR) Inpatient Service: Deliver the pre‑hospital discharge component of the CR service (Phase 1). Provide information on diagnosis, investigation results, and education about risk factors, lifestyle modification, chest pain management, driving advice, returning to work, medications and exercise. Offer psychological support, coaching and signpost to other services. Service delivery aligns with BACPR and NICE recommendations.
* Cardiac Surgery Liaison Service: Offer support and education for cardiac inpatients and families from surgical acceptance to transfer. Liaise with surgical centres and the RCHT team; coordinate timely surgical work‑up.
* Cardiac Rehabilitation Exercise Programme for Truro and Helston: Support exercise/education groups alongside the community cardiac team, in line with BACPR standards. Sessions are held off‑site in local gym facilities; the successful candidate must be able to travel to these venues.
