We are looking to recruit an experienced Practice Nurse with Chronic Disease Management Qualifications to join our nursing team. The hours on offer are 28 hours per week between 08.30-18.30 Monday to Friday. The specific hours are to be confirmed. Bedworth Health Centre is a large training practice in the heart of Bedworth Town Centre.
Main duties of the job
The candidate will hold the necessary qualifications to undertake chronic disease management, childhood immunisations, and cervical smears as a minimum.
1. Cytology
2. Routine immunisations / Childhood immunisations and ensure vaccines are administered under patient group directions.
3. Travel Clinic
4. Perform Ear Irrigation.
5. Perform ECG recordings and present results to the requester.
6. Undertake chronic disease management (e.g., asthma, diabetes, coronary heart disease, hypertension) within Practice protocols, including monitoring progress and response to treatment.
7. Assist GPs with minor surgery and coil fittings.
8. Administer Depo injections and carry out pill checks.
9. Request pathology tests, e.g., urine culture, swabs.

Bedworth Health Centre is the largest GP practice in North Warwickshire with a patient list size of 21,500 patients. You would be joining a very supportive team of 14 G.Ps, 4 Practice Nurses, 1 Trainee ACP, 1 Practice Paramedic, 2 Assistant Practitioners, and a Health Care Assistant.

10. Chaperoning and assisting patients where appropriate who are being examined by another clinician.
11. Assist GPs with minor surgery and coil fittings.
12. Administer depo injections and carry out pill checks.
13. Request pathology tests, e.g., urine culture, swabs.
14. Interest in Diabetes or Coronary Heart Disease and Contraception.
15. Follow agreed clinical protocols with referral to senior nurses or GPs as appropriate.
16. Maintain and clean equipment used by the nurses and GPs.
17. Maintain GP and Nurses rooms, stocking and rotating items as required.
18. Maintain stock of waiting room leaflets as instructed by partners and Practice Manager.
19. Order Health Promotion Literature.
20. Able to recognize and manage anaphylaxis according to current UK guidelines.
21. Order stock from the hospital.
22. Order vaccinations to maintain stock levels.
23. Participate in administrative systems in the Practice.
24. Maintain accurate records at all times.
25. Advise senior nurses of potential problems or errors within the range of assigned tasks.
26. Attend and participate in any Practice meetings when required.
27. Occasionally undertake home visits.
28. Perform any other delegated duties appropriate to the post.
Person Specification
Qualifications
1. Registered General Nurse with professional registration with RCN & NMC.
2. Cytology
3. Childhood Immunisations
4. Diplomas in main chronic disease areas, Respiratory and Diabetes.
Experience
1. Experience of working in General Practice and able to undertake Cervical smears and administer childhood immunisations.
2. Diploma in Chronic Disease Management.
3. Experience working in out-of-hours, extended access, etc.
