This role is responsible for the delivery of evidence-based nursing services, working as part of the practice multi‑disciplinary team, delivering care within their scope of practice to the entitled patient population.

Working autonomously, the Practice Nurse will be responsible for a number of clinical areas such as public health and screening programmes, including infection prevention and control, chronic disease management, immunisations, health promotion and prevention, well man/woman clinics and cervical screening. They embed population health management approaches within general practice, targeting health inequalities and improving access.

The post‑holder will be an integral part of the general practice team, as well as supporting the wider practice nurse network and multi‑disciplinary team. They are to participate in formal training events and to promote best practice in their area of expertise as well as supporting the practice management team in the reviewing and delivery of clinical policy and procedure.

Our practice is led by a team of 4 dedicated partners and 3 salaried GPs and supported by a wide‑ranging multidisciplinary team that includes a Clinical Pharmacist, Practice Nurses.

This robust clinical workforce is underpinned by a highly skilled and efficient administrative team that ensures the seamless operation of the practice. Together, our multidisciplinary team is committed to providing outstanding care and continually improving health outcomes for the communities we serve.


Job responsibilities

The Practice Nurse plays a key role in delivering safe, effective, and evidence‑based nursing care within general practice. Responsibilities include:

* Infection Prevention and Control (IPC)
o Support the IPC Lead and work in accordance with the practice IPC Policy.
* Patient Assessment and Consultation
o Undertake holistic assessments of patient needs through consultation with patients and, where appropriate, their families and carers.
* Long‑Term Condition (LTC) Management
o Contribute to the management of long‑term conditions, participating in quality improvement and assurance initiatives.
* Care Management for Frail and Housebound Patients
o Develop, implement, and embed effective care management programmes for frail, elderly, and housebound patients.
* Public Health, Screening, and Health Promotion
o Support public health and screening programmes, including immunisations, vaccinations, and cervical screening.
o Provide opportunistic health promotion advice to patients.
* Population Health Management
o Embed population health approaches within practice to reduce health inequalities and improve patient access.
* Evidence‑Based Clinical Care
o Deliver routine nursing care in line with current evidence‑based practice, national guidance, and local protocols, including those from NICE and relevant National Service Frameworks.
* Education and Support
o Support and mentor new‑to‑practice nurses and other healthcare professionals.
o Educate and empower patients and their families to self‑manage their conditions.
* Clinical Audit, Research, and Governance
o Participate in clinical audits and research activity.
o Support the development, review, and updating of practice protocols, guidelines, and procedures.
o Contribute to the full spectrum of clinical governance.
* Working to Protocols and Scope of Practice
o Work within national and local protocols.
o Recognise professional boundaries and refer patients appropriately when required.
* Enhanced and Additional Services
o Support delivery of enhanced services and other commissioned service requirements.
o Lead public health campaigns (e.g. flu clinics).
o Provide travel health advice services.
* Safeguarding and Legal Compliance
o Adhere to current legislation and practice policies relating to Safeguarding Adults and Children.
* Incident Management and Complaints
o Participate in the management of patient complaints when required.
o Contribute to learning from incidents and near‑miss events.
* Mandatory Training and Appraisal
o Complete all mandatory training and induction requirements.
o Participate in annual appraisal and ongoing performance review.
* Clinical Record Keeping and Coding
o Maintain accurate, contemporaneous clinical records in line with legislation and best practice.
o Use SNOMED CT coding effectively.
* Practice Targets and Registers
o Contribute to achievement of practice targets, including the Quality and Outcomes Framework (QOF).
o Maintain chronic disease registers and effective call/recall systems.
* Clinical Skills and Direct Patient Care
o Provide wound care, including ulcer care and Doppler assessments.
o Chaperone patients when required.
o Support the team in managing clinical emergencies.
* Teamwork and External Liaison
o Liaise with external services and agencies to ensure appropriate patient support, particularly for vulnerable patients.
o Maintain a clean, safe, and effective working environment at all times.

Additional Duties The Practice Nurse may be required to undertake additional duties appropriate to the role, depending on service needs, workload, and staffing levels.


Person Specification


Experience

* Experience of chronic disease management
* Experience of working in a primary care environment
* Experience of working as a practice nurse


Qualifications

* Registered Nurse Post graduate diploma or degree (other)
* Travel Medicines Immunisations (routine, childhood and travel)
* Women’s health (cervical cytology and contraception etc)
