This is an exciting opportunity for an Assistant Practitioner to work as part of our multi-disciplinary Integrated Neighbourhood Teams, working within the community therapy team. By joining us, you will have many opportunities to develop your clinical skills, physical assessment, and clinical reasoning, with the support of the wider team. You will work alongside Physiotherapists, Occupational Therapists, Community Support Workers, and the wider multidisciplinary team (MDT) to support people in facilitating safe hospital discharges and preventing admissions where appropriate and safe to do so. You will also work within our planned workstream to promote self‑management and support individuals to maintain their independence within the community.
Responsibilities
* Provide holistic assessment and treatment to patients on the caseload with the guidance of the wider team.
* Maintain a high professional standard of care in adherence to all protocols, policies and guidelines.
* Manage a caseload within your scope of practice and with the support of the wider team.
* Provide clinical assistance to other members of the clinical team.
* Maintain accurate electronic clinical records.
* Respond to referrals across urgent and planned pathways, including hospital discharge facilitation.
* Proactively monitor and manage patients on the caseload to ensure the best outcomes for them.
Qualifications – Essential
* Successful completion of a relevant Level 5 Foundation Degree or an equivalent qualification, with demonstrable knowledge and experience at degree level.
* Experience of working in a health care or social care setting providing direct patient care.
* Ability to communicate in varied ways with colleagues, service users and other stakeholders.
* Demonstrate knowledge and understanding of Clinical Governance and Clinical Risk.
* Able to evidence experience of working autonomously, and the ability to seek appropriate advice or guidance from senior staff.
* Evidence of effective time management and caseload management.
Desirable
* Community work demonstrating a range of clinical experience.
* Evidence of MDT working and clinical escalation.
