Grovelands Medical Centre is seeking an enthusiastic Practice Clinical Pharmacist with General Practice experience to collaborate closely with our practice teams and deliver our pharmacy vision: providing excellent pharmacy-led services for registered patients. Experience in General Practice is desirable.
Qualifications: Registered with the GPHC, Degree or equivalent in pharmacy, Clinical Diploma, Previous primary care experience.
Main duties of the job
* Maintain up-to-date clinical knowledge, understand primary care prescribing, conduct medication reviews, and develop strategies for improving prescribing.
* Uphold NHS commitments to addressing health inequalities and ensure care is accessible to all communities.
* Recognize personal and colleagues' limitations, refer appropriately, and work effectively in a flexible, autonomous environment.
* Exemplify excellent communication skills, influence others, and manage clinical caseloads in primary care, both face-to-face and remotely.

* Manage repeat prescriptions, acute requests, and medicines transfer processes including discharge letters and medicines reconciliation.
* Provide telephone clinics for medication queries and concerns.
* Perform structured medication reviews and manage aspects of the QOF and enhanced services.
* Develop systems for safer prescribing and foster relationships with local pharmacies and the PCN pharmacy team.
* Participate in meetings for prescribing and patient safety review, and engage in schemes like Prescribing Quality, PINCER, and audits.
