Community Care is hiring a Case Manager/Care Manager to join our Family Care Program team.
This rewarding full-time opportunity is perfect for someone who enjoys team collaboration, community case management, and being part of a mission-driven organization. Community Care’s mission is to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community. The position is based in Kenosha County and offers a weekday schedule along with a $5,000 SIGN-ON BONUS.
Care Manager/Case Manager Job Responsibilities:
1. Manage an assigned caseload of program members in collaboration with an RN Care Manager.
2. Complete comprehensive member assessments, care plans, and functional screens.
3. Provide ongoing care coordination and quality assurance monitoring of services.
4. Maintain required documentation in accordance with established standards.
Care Manager/Case Manager Job Requirements:
1. Education: Bachelor’s Degree in Human Services or a related field required. A minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities may substitute for a non-human services degree.
2. Experience: One year of experience with older adults or persons with disabilities in a social service capacity preferred.
3. Certification: Eligibility for long-term care functional screener certification required.
4. Travel: Regular local travel requiring the use of a personal vehicle, valid driver’s license, and proof of insurance coverage.
Care Manager Job Schedule:
* Monday to Friday, 8:00 AM - 4:30 PM
