What the Job Involves Joining the Rhondda Cynon Taf East Learning Disability Team, you will work in the East of the borough, covering areas from Pontypridd to Aberdare/Hirwaun. The role involves high-quality social work practice in assessment, care planning, and review. You will conduct assessments according to the Social Services and Well-Being Act (Wales) 2014, develop and review care and support plans, and undertake Mental Capacity Assessments in line with the Mental Capacity Act 2005. You will assess risks to promote independence and choice and collaborate with other agencies using a multi-disciplinary approach. This position requires maintaining up-to-date electronic records and traveling independently within the service area. Team members primarily work remotely, with one office day a week.
