About the Role
We are seeking a Band 7 Palliative Care Clinical Nurse Specialist (CNS) in Pembrokeshire. This is an excellent opportunity for an experienced specialist palliative care nurse to develop and enhance local services, delivering high-quality, patient-centred care for those with life-limiting conditions.
Key Responsibilities
* Provide expert clinical assessment, symptom management and psychological support.
* Act as a key contact for patients, carers and professionals, offering advice, education and coordinating care across settings.
* Work closely with multidisciplinary teams to support seamless care pathways and promote best practice.
* Deliver specialist training across primary and secondary care, and contribute to audit and quality improvement initiatives.
* Assess, plan, implement and evaluate evidence-based nursing care and provide expert advice and clinical practice.
* Manage a team of specialist nurses, providing specialist education and training.
* Work collaboratively with clinical colleagues to develop services and improve the quality of care delivered.
* Lead the development of clinical services within the specialist field.
* Demonstrate the Health Board’s values at all times.
Qualifications & Experience
* NMC-registered with substantial specialist palliative care experience.
* Advanced clinical knowledge and skills.
* Strong leadership and advanced communication skills.
* Emotional intelligence and resilience.
* Ability to work independently and collaboratively.
* Desirable: Ability to speak Welsh.
