Service Coordinator - Tri-Borough
Location: Chelsea and Westminster, Charing Cross and St Mary’s hospitals, London
Salary: £23,590 to £25,155 (plus Inner London Weighting, £3,344, if applicable) per annum
Hours: 35 per week
Fixed Term Contract (until 30th September 2026)

Step into the impactful role of a Service Coordinator as part of a dedicated team committed to making a real difference. You will support individuals with unmet social needs, loneliness, and seclusion, creating bespoke support plans to foster independence and well-being. This role offers an opportunity to make a meaningful impact through empathy and persuasion, spreading kindness and creating lasting change.
What will a day in the life of a Service Coordinator involve?
* Meeting with service users, assessing their needs, and guiding or signposting them to connect with groups or manage health conditions to improve their well-being.
* Liaising with partnership agencies to promote the service and build meaningful links.
* Producing and maintaining monthly reports.
What do you need to be a successful Service Coordinator?
* Good communication skills.
* Sound knowledge of Health and Social Care services.
* Proven ability to deliver high-quality public services.
