Overview
An exciting opportunity to join a new team operating in the community across the London Borough of Hillingdon. We are seeking a nurse to support a multidisciplinary team to engage with the homeless population who struggle with substance use within the borough of Hillingdon. The team uses holistic assertive outreach techniques to support service users to access community-based drug and alcohol treatment and to secure long-term accommodation. The role will be part of the Addictions Recover Community, Hillingdon (ARCH) team, based in Uxbridge, but predominantly community-facing, working in sites across the borough. You will be a registered nurse (RMN/RGN) with a keen interest to work within the field of addiction psychiatry. The successful candidate will demonstrate an ability to engage with hard to reach individuals to provide treatment within a variety of community-based settings.
You will have a range of therapeutic skills to support service users such as motivational interviewing techniques to encourage service users to work towards recovery. Knowledge of dual diagnosis, physical health and assessment skills would be beneficial.
The role involves assessing and managing substance-misusing clients by applying harm minimisation approaches, relapse prevention, motivational interviewing, detoxification programmes according to clinical needs, and in line with up-to-date evidence-based practice and relevant Trust clinical protocol and policies.
Our service is part of CNWL Foundation Trust, a provider offering a wide range of NHS drug and alcohol treatment interventions to diverse populations. Our substance misuse services include community services, A&E liaison services, peripatetic nurses, family therapy, a National Problem Gambling Service and a Club Drug Clinic. We are committed to reducing harm and supporting service users and families toward recovery, and to working with partners and local providers.
We encourage training and development of nurses’ skills and support learning in areas such as motivational interviewing, relapse management, and clinical skills including venepuncture, ECG, dry blood spot testing and fibro scan training.
Responsibilities
* Assess and manage service users with substance misuse using harm minimisation, relapse prevention, motivational interviewing, and detoxification programmes as clinically indicated.
* Provide specialist drug and alcohol outreach clinics for local hostels and homeless individuals; coordinate health services within the local authority catchment.
* Work as a member of a multidisciplinary team to deliver management and treatment for clients with drugs and/or alcohol issues; include care planning, general health and wound assessments, injecting site assessments, lifestyle and physical health assessments, and urine drug screening.
* Utilise mental health assessment skills and refer to appropriate mental health services; contribute to psychosocial group work.
* Promote health and wellbeing, perform health checks, provide early intervention, signposting, and referrals to other health services.
* Develop and maintain pathways to specialist services to ensure health needs are met, including primary care, and signpost service users to appropriate settings.
* Manage a caseload of service users accessing rough sleeper support; ensure safety and escalate risks where required; provide holistic care for individuals accessing addictions support.
Qualifications
* Registered nurse (RMN/RGN) eligible to practice.
* Interest in addiction psychiatry and ability to engage hard-to-reach individuals in community settings.
* Therapeutic skills including motivational interviewing; knowledge of dual diagnosis, physical health, and assessment skills.
* Experience or willingness to develop skills in relapse management and clinical techniques such as venipuncture, ECG, dry blood spot testing, and fibro scan training.
