Overview
Hospital at Home site Victoria Hospital. The Hospital at Home Teams within Fife provide a service for the frail elderly population (predominantly over 65yrs), which prevents acute hospital admission or facilitates early supported discharge where acute hospital admission has taken place. It allows people to have a period of “acute” medical care within their own home, which will in turn allow the person to recover from a period of illness without needing hospital admission.
An exciting vacancy has arisen to join our H@H service in Fife. This post is part of the core Hospital at Home Service delivering in-reach work within Victoria Hospital. This will involve being based within the acute hospital working closely with Frailty Team and Medicine of the Elderly Consultants. As part of the wider H@H service the post will include a rotation between in-reach and community H@H teams. This is a relatively new aspect of our Hospital at Home service and continues to evolve with the enthusiasm and initiative of the staff in post. The role focuses on supporting discharges and carrying out clinical assessments and ensuring a smooth transition for patients stepping down to the Hospital at Home Service as well as educating colleagues as to the role of the service.
Responsibilities
* Deliver a high standard of safe and effective acute medical patient care within mainly the acute setting but also supporting the wider Hospital at Home service in the community as required.
* Work autonomously in this evolving aspect of the service and participate in a rotation between in-reach and community H@H teams.
* Support discharges and carry out clinical assessments to ensure a smooth transition for patients stepping down to the Hospital at Home Service.
* Educate colleagues as to the role of the Hospital at Home service.
* Travel within the area in a timely manner to support the wider Hospital at Home service and respond to deteriorating patients with daily routine clinical interventions in the community.
* 7 day working is required; shifts 08:00–18:30 and base location is Victoria Hospital with the H@H main service operating from 08:00–22:00. A minimum initial period of 2 months will be based within the community H@H teams to build knowledge and skills prior to working within the acute site.
Qualifications and requirements
* Must have a degree level qualification in clinical decision making and the desire and ability to work towards an NMP qualification.
* Additional skills required: venepuncture, ECG, IV additives and cannulation.
* Excellent communication, confidence, a good degree of assertiveness and time management skills are essential.
* Ability to travel within the area in a timely manner to support the wider Hospital at Home service and respond to deteriorating patients.
* Legal entitlement to work in the United Kingdom. If not a UK or Irish national, right to work verification is required. Pre-employment checks and PVG status may apply; sponsorship may be possible for eligible posts with licenced sponsors and meeting salary thresholds.
