Role Overview
We offer a £2000 welcome payment to qualified and experienced Social Workers (grade J and above) who are new to Norfolk County Council. The payment must be repaid in full if you leave within two years of employment. For experienced Social Workers new to Norfolk County Council, we promise to match or exceed your existing salary, provided it falls within the advertised grade spectrum.
We are seeking an experienced Specialist Adult Social Worker (Level 2) to join our established Neuro Rehabilitation team at Caroline House. This unique and rewarding opportunity involves supporting adults recovering from acquired or traumatic brain injuries and neurological conditions at a pivotal stage of their rehabilitation journey. You will be based at Colman Hospital in Norwich and may be required to take on community‑based cases for patients with neurological disorders.
Responsibilities
* Complete comprehensive, strengths‑based assessments under the Care Act 2014.
* Apply the Mental Capacity Act, supporting individuals with fluctuating capacity and contributing to best‑interest decisions.
* Lead on safe, timely and person‑centred discharge planning, working closely with hospital teams and community services.
* Hold a varied caseload of in‑patient rehabilitation cases and some community‑based adults with neurological conditions.
* Support families adjusting to sudden or life‑changing injuries, helping them understand the rehabilitation process and future options.
* Identify safeguarding concerns and balance risk with autonomy, recovery, and personal outcomes.
* Advocate for individuals' rights, wishes, independence, and dignity.
* Work consistently within the principles of Community‑Led Support (CLS), enabling creative, community‑based solutions.
Qualifications & Experience
* Registered with Social Work England as a practising Social Worker.
* Be confident working with adults with physical disabilities, neurological conditions, or older people.
* Have experience managing hospital and/or community caseloads.
* Be skilled in MCA assessments, safeguarding, and complex case coordination.
* Have strong organisational skills and professional judgement.
* Thrive in a fast‑paced, collaborative, multidisciplinary environment.
