The successful candidate will join our experienced multidisciplinary team of skilled and highly motivated staff in our 24-bedded inpatient community ward. We deliver quality services for patients requiring generic rehabilitation for older persons, deliver frailty, subacute and discharge to assess pathways. This opportunity will allow you to experience a holistic view of community services and gain knowledge of the full range of health and social services available for patients, both within hospital environment and other services in the community that can help the patient stay in their preferred discharge destinations safely.

An opportunity for leadership and clinical supervision is provided to all ward staff, enabling the maintenance of professional standards within NMC guidelines to provide a high standard of patient care. You will work alongside a multi-disciplinary team of Doctors, ANPs, Therapists, Nursing staff, Pharmacists, and support workers to provide high quality care to patients on our ward.


Main duties of the job

To promote and maintain a high standard of individualised care by actively contributing to the planning, delivery, and evaluation of care, ensuring that care plans are used effectively, adhered to, and updated as the patient's condition dictates.

To maintain a safe, welcoming environment for patients, relatives, visitors, and staff.

To demonstrate clinical decision-making skills, displaying evidence-based practice

To deliver patient care using a variety of clinical skills in line with competency framework.

To be actively involved in promoting self-care for patients and building health promotion into care as appropriate.

Job responsibilities

The must haves for this role:

Registered Nurse /Adult Nurse Trained with up-to-date NMC registration

To work bank holidays/weekends or unsocial hours to meet the needs of the service.

Reasonable level of fitness required.

Person Specification


Education/Qualifications /Training

* Registered Nurse
* Evidence of ongoing professional development


Continuous Professional Development

* Desire to undertake further training and self improvement to support patient experience


Previous Experience

* Community, Medical Nursing, Rehab & Palliative Care experience
* At least 1 years post qualifying experience


Knowledge, Skills & Abilities

* Well developed written and verbal communication skills
* Able to demonstrate use of own initiative.
* Evidence of effective team working
* Able to demonstrate knowledge of public health and promotion of self care
* Teaching relatives and carers across all areas of care
* Ability to participate in standard setting and audit
* Ability to demonstrate organisational skills
* Ability to demonstrate and show evidence of effective clinical skills
* Ability to identify risks
* Ability to work autonomously
* Ability to manage distressed patients or relatives with terminal/palliative/ Dementia care needs.
* Ability to manage patients who have intravenous therapy and complex dressings
* Ability to identify risks and act accordingly with regard to safety of oneself, patients and others using the appropriate aids
* Experience of delivering undergraduate student supervision and clinical education to graduate standard in area of speciality
* To work bank holidays/weekends or unsocial hours to meet the needs of the service.
* Reasonable level of fitness required.

£32,602 to £39,686 a yearper annum (incl of HCAS)
