Overview
The Trust supports the community and helps patients remain independent in their own homes. You will actively assist the Community Nursing Team in providing nursing care and support to patients and families in the community setting, including end‑of‑life care.
Key Responsibilities
* Assist the Community Nursing Team with nursing care and support to patients and families in the community setting, including end‑of‑life support.
* Work evenings and overnight shifts from 19:00 to 08:00, 20 hours per week across 4 evenings, covering weekends and bank holidays.
* Provide treatment to a range of patients with varying health and social care needs, working in pairs for safety and supported by the Team Leader and team members.
* Collaborate closely with the multidisciplinary team (Social Workers, GP's and Allied Health Professionals) to ensure coordinated patient care.
* Maintain a full, clean driving licence and have access to a vehicle for service duties.
Qualifications
* Full, clean driving licence with vehicle access.
* Relevant nursing qualification at Band 3 level or equivalent, with a commitment to compassionate, patient‑centred care.
Compensation & Benefits
