About the role
The main purpose of this post is to provide physiotherapy input, as part of a Specialist Multi-disciplinary Team, to patients referred for specialist palliative/end of life care. The post holder will plan and deliver holistic care to patients, their families and carers on the physiotherapy caseload, both in the community and in Hospice settings. There is also a focus on delivering our breathless programme, providing education and exercise to patient groups. You will provide evidence-based physiotherapy interventions to patients following HCPC clinical standards of care and CSP professional Codes of Conduct.
Responsibilities
* Provide responsive, clinical physiotherapy to patients under the care of the Hospice, in accordance with priorities set by the Director of Clinical Services.
* Work as an autonomous practitioner with professional and legal accountability for all aspects of own work.
* Triage referrals for physiotherapy and manage a clinical caseload of patients with palliative care needs.
* Holistically assess, plan, develop and provide evidence-based physiotherapy intervention for patients with complex physical, psychological and spiritual needs.
* Identify patient needs and refer to relevant agencies or professionals as appropriate.
* Utilise assessment and analytical skills to diagnose and select appropriate treatment options.
* Evaluate patient progress, reassess and adjust treatment plans; liaise with the multidisciplinary team.
* Identify needs for specialist equipment, order and educate on use to patients and carers.
* Collaborate with primary and secondary care to optimise management and act as a specialist resource for generalist staff.
* Provide intervention in patient’s preferred place of care with emphasis on empowerment, enablement and self-management.
* Communicate complex information clearly; facilitate understanding and manage barriers to intervention.
* Attend and contribute to multidisciplinary meetings; support self-management education programmes.
* Proactive discharge planning and follow-up to ensure optimal function and quality of life.
* Attend inpatient unit meetings and liaise with Hospice at Home, Clinical Specialist Nurses, Social Care and Community teams.
Main duties and professional practice
* Clinical patient services: provide autonomous, responsive physiotherapy to patients; triage referrals and manage caseloads.
* Service development: share knowledge with the healthcare team; CPD and student education; participate in MDT meetings.
* Policies, procedures and audit: contribute to local policies, clinical governance, and audits; understand complaints procedures and incident reporting.
* Communication: obtain consent, discuss interventions, and communicate with patients and carers; understand capacity and consent frameworks; represent physiotherapy in MDTs.
* Freedom to act and autonomy: work as an autonomous practitioner within professional guidelines; manage own schedule and priorities; collaborate and delegate appropriately.
Requirements
* Qualifications – Essential: Honours degree in physiotherapy; HCPC registration; CSP membership; CPD portfolio; commitment to CSP & NHS codes of conduct.
* Experience – Essential: Postgraduate experience across diverse physiotherapy specialties; ability to work in a multidisciplinary team; supervision experience of assistants/students; autonomous practice; strong clinical governance and IT literacy.
* Desirable: Communication skills training; Accredited Clinical Educators course; postgraduate education and involvement in audits or research.

* Full UK driving license and access to a vehicle; commitment to professional development.
