Job Summary: We are looking for an enthusiastic, driven registered practitioner who can work autonomously in the community, complete full mental health assessments, manage risk in the community, while also being an integral part of the multidisciplinary team. Responsibilities
Main duties of the job
* Provide comprehensive assessment of a service user’s mental state and social needs to aid the completion of a collaborative care plan for community management.
* Provide comprehensive risk assessment, formulation, and management as part of the service user’s journey.
* Act as keyworker for a small caseload of service users, with support from the multidisciplinary team.
* Cover the duty/shift coordination role on a regular basis.
* Liaise with other agencies involved in a service user’s care.
* Support and supervise junior members of staff.
* Lead on discharge or transfer of care for service users to appropriate agencies.
* Adhere to professional confidentiality standards and the codes of practice of your registering body.
* Discharge professional responsibilities under the Mental Health Act (1983) and Mental Capacity Act (2005) and the NMC Professional Code of Conduct.
* Be accountable for own practice and manage medicines in accordance with NMC guidelines and the Trust Medicines Policy.
* Maintain knowledge of legal requirements affecting care (Mental Health Act, Mental Capacity Act, Care Act, Safeguarding).
The post holder will be based in the Home Treatment Team (CHTT) and be the first point of contact for all referrals. The role includes urgent response to requests for assessment and treatment of people experiencing a mental health crisis.
You will work autonomously as well as as an effective team member.
Qualifications & Experience
Essential
* RMN (current NMC registration)
* Evidence of continuing professional and personal development
* Full and current driver’s licence and car owner (or willing to participate in the Trust car lease scheme)
Desirable
* Mentorship, ENB 998 or equivalent
* RGN
* Diploma in cognitive behavioral therapy/psychodynamic counselling
Experience:
* Demonstrable experience of working with people who experience serious ongoing mental health problems in a community or outpatient setting post qualification
* Experience of working in a multidisciplinary team
* Experience of assessing and managing crisis with those with a severe mental illness
* Experience of clinical supervision

Salary: £44,485 to £52,521 per annum inclusive of Outer London HCAs
