Patient Navigator/Administrator - Specialist Palliative Care Team

This is an exciting opportunity to work within the specialist palliative care team across MBHT, based at Westmorland General Hospital. A Barrow Base will be considered if preferred; please note this on your application.
Role Responsibilities:
1. Ensure timely appointments and support all aspects of the patient pathway.
2. Support outpatient clinics, including transcribing consultation letters.
3. Assist with patient referrals to agencies such as local hospices, cancer care, and Citizens’ Advice.
4. Liaise with and support patients and relatives under the supervision of the specialist nurses or consultants.
5. Coordinate the service's daily operations and manage data collection and analysis.
6. Work with team members to ensure timely care and optimal service delivery.
7. Support administrative tasks related to patient pathways, including booking appointments, transcribing outpatient letters, and supporting data collection and referrals.
The role combines administrative duties with patient support, working closely with the Team Lead. It requires initiative, confidentiality, and a focus on high-quality patient experience.
Skills and Requirements:
* Proficiency in Office 365 (Excel, Word) and data management.
* Excellent communication and organizational skills.
* Ability to work independently and as part of a team.
Additional Information:
The post involves working across multiple hospitals and community healthcare facilities in the Morecambe Bay area, including Furness General Hospital, Royal Lancaster Infirmary, and Westmorland General Hospital.
