Weare looking to recruit Digital and Transformation Lead to drive a new visionand help develop and transform new and existing services across the PCN.
Therole forms an integral part of the leadership team working alongside the PCN ClinicalDirector, Board and Senior Management Team to deliver the PCN strategy,identifying and implementing service transformation and innovation. The Digitaland Transformation Lead will be responsible for managing agreed projects with afocus on developing strong relationships and partnerships across the healthcaresystem.
As an established single practice PCN with apatient population of 43000. We already have a high functioning leadership teamoverseeing all practice activity and business. This means the post holder willnot have to negotiate difficult barriers between member practices to affectchange across our 6 sites.
Therole has an outward focus and will support the PCNs resilience by activelyseeking development opportunities. Thisis a exciting role and would suit someone with enthusiasm and drive and apassion for improving outcomes for patients.
Main duties of the job
The role of the PCN Digitaland Transformation Lead is to support increased access to care for patients across our large fullymerged PCN. You will lead the adoption and/or optimisation of newtechnology and other initiatives to improve the care offer and enabling PCNstaff to work more effectively to support the sustainability of generalpractice services.
You willsupport the Clinical Director and engage with a wide range of stakeholders, utilisingpopulation health data to transform primary care services and collaborativeapproaches to service development and delivery.Developing strong partnerships and actively supporting integrated neighbourhoodways of working.
You will be working on the implementation ofidentified projects, as agreed by the Clinical Director and Board, supportingthe delivery of the NHS Long Term Plan and Network Contract Des and deliveringlocal objectives and priorities.
The role will require good analytical skillsand numeracy, knowledge of the NHS and Primary Care.

KeyResponsibilities:
The PCNDigital and Transformation Lead will play a pivotal role in the development andcontinued evolution of Beacon Medical Group to support the delivery of high-qualityprimary care services. Working with theclinical and operational teams to identify opportunities to make positivetransformation, and provide the inspiration, resource, and tools to executechanges in a collaborative way.
MainResponsibilities:
To support the clinical director and Board inthe delivery of the PCN Contract DES.
To provide leadership and support for anagreed portfolio of improvement and service transformation projects
Support the PCN Clinical Director and Board withthe writing of business and development plans
Act as a point of expertise, translatenational and local policy into meaningful changes, and will support the PCN toachieve improved outcomes for the local population.
Support the Clinical Director and Board, toensure all relevant national strategies, policies, targets and frameworks arefully implemented
Lead on transformation and innovation ofservices and digital projects across the PCN
Improve adoption and optimisation of newtechnology to enhance patient access and experience and increase PCNproductivity
Attend cross organisation meetings as required Collaborative Board, Local Care Partnership
Deliver projects to support the PCN andimprovements to health outcomes for the patient population
Write and submit project reports for the Boardas required, showing progress against milestones, status, issues and risks
Oversee the roll out and delivery of newservices, working alongside the Head of Operations and Operations Managers
Be a champion for best practices, learningfrom experience and from others, supporting the spread of innovation within thePCN
Develop programmeand project management tools, reports and templates, that are transferablebetween projects, and can be used for the whole project lifetime, from designto delivery.
Manage delegatedbudget in line with organisational procedures

Transformation:
Develop plans for projects identified by thePCN board, utilising the support of the PCN Business Analyst and OperationsManagers and support the implementation of the projects
Work with the PCNBusiness Analyst and Operations Manager to ensure the PCN has a full suite ofdata that can be interpreted and utilised for improvement, and driving forwardthe PCNs strategy to support population health management
Build relationships and facilitatecollaboration between practices and the wider system to support the delivery ofcare to patients
Engage with the ICB Primary Care and Digital teams to ensure localtransformation plans and activity are aligned to the ICB priorities improving access, PCN Development.
Provide feedback to the management team on progress of currentinitiatives, capacity for change, and issues so that plans can be adapted inresponse
Support a programme of audits and surveys thatsupport, and evidence measures of improvement
Understanding the PCN finances, in liaisonwith Director of Finance and monitoring PCN Des and other PCN income streams
Undertake full evaluation cycle of newservices and service improvement
Digital:
Have management oversight of theupkeep of our website to ensure key messages are shared and patient access andnavigation of the website is well planned (supported by our Patient Liaisonand Communications Lead).
Improve adoption and optimisation of new technologyto enhance patient access and experience and increase PCN productivity
Review and improve the PCNs digital maturityby ensuring we are using safe up to date systems that will improve ourproductivity and access for patient care
Support increased access to care for patients, by supporting theadoption and of new technology and other initiatives to improve the care offerand enabling practice staff to work more effectively to support thesustainability of general practice services
Training:
Work collaboratively as a key member of thePCN team, help develop and promote a positive working culture encouraging staffparticipation and involvement in developing their own contribution towards thesuccess of the organisation
Offer support and training if required toleads, managers and clinicians to help support their workload
Communication:
Buildrelationships and facilitate collaboration between PCNs, practices and thewider system to support delivery of care to patients, maintaining credibilitywith key contacts and system colleagues
Represent the network at meetings, in thecommunity as required.
Translate and present information into clearand appropriate formats, such as written documents and presentations to ensureunderstanding across a range of audiences
Be responsible for sharing information andbest practice and persuading and influencing change in services with cliniciansand a range of other stakeholders.
The information which the post holder will berequired to communicate will be of a highly complex, sensitive, and potentiallycontentious nature. The post holder will need to use persuasive andmotivational skills to achieve improvements in services where there isresistance to change.
Management:
Supportworkforce development planning in relation to new services, serviceimprovements and service redesign
Work withclinical leads on the management and development of clinical ARRS staffincluding: Workforce development including the review of population healthneeds and skill mix assessments to identify future workforce needs, supporttraining framework developments in line with the PCN contract Des
