We are currently seeking an experienced Leaving Care Personal Advisor to work with our local authority client in Cardiff. The role involves supporting young people aged 16-25 who are leaving care, helping them transition to independent living and achieve positive outcomes.
Key Responsibilities
Work with young people to create and review Pathway Plans.
Provide advice, guidance, and practical support around housing, education, employment, and health.
Help young people develop life skills and confidence to live independently.
Liaise with social workers, housing providers, education and training services, and other key partners.
Keep accurate case notes and reports in line with local authority policies and procedures.Key Requirements
Previous experience working with care leavers or vulnerable young people.
Strong understanding of leaving care legislation and safeguarding.
Excellent communication and organisational skills.
Ability to build trusting relationships with young people.
Enhanced DBS check (or willingness to obtain one).
Relevant qualification in social care, youth work, or a related field preferred.
Contract Length: 3-6 months with a view to extend to an ongoing contract.
Pay Rate: £16-£21
Working pattern - 37 hours per week.
