Offer description

Overview

The post-holder will be based with the rest of the team and provide middle grade medical input to the team. This role is led by a 0.8 Less than full time consultant psychiatrist who offers clinical support and advice to all practitioners around medical treatment, formulation of cases and developing appropriate treatment plans and will provide clinical and managerial supervision to the successful applicant.

The Home Treatment Team is an integral part of Adult Acute mental health services within Pennine Care NHS Foundation Trust. The team offers intensive community based support to those service users who are experiencing an acute episode of mental ill health as an alternative to admission to an inpatient bed. The team also provides intensive support to service users who are leaving a hospital bed as part of an individual's discharge pathway. Where admission is unavoidable, the team facilitates discharge at the earliest opportunity. This may involve planned supported leave prior to discharge.

The main function of the team is to provide high intensity treatment and crisis management away from an inpatient setting. The team also offers a supported discharge pathway and provides discharge follow up for individuals who are not going to receive longer term support from mental health services.

The service works with patients aged 18 years to 65 years (can be older if under GA services). The post-holder will work closely with other team consultants inside and out of the area. There are close working relationships with the Liaison Mental Health Services, GA Wards, CMHT, OPD Psychiatry, EIT, Living Well Team in the Stockport Team.

The post holder will be expected to offer a senior level of support for the team with the consultant in HTT and close working with the other members of the medical team and nursing staff/support workers.
