We are seeking to recruit enthusiastic, highly motivated Band 5 Occupational Therapists to work within our Integrated Neighbourhood Team (INT) in Weston. The Sirona Integrated Neighbourhood Teams (INT) are innovative and dynamic. We provide active rehabilitation to patients in their homes to maximize their functional potential and support independence. Our therapists assist with hospital discharges, admission avoidance, and long-term self-management, helping patients live better lives at home.
You will work within a friendly and supportive team alongside Therapists, Assistant Practitioners, Nurses, Nurse Associates, Advanced Clinical Practitioners, Community Support Workers, and Administrators. We offer a flexible service covering extended hours, 7 days a week.
Key Responsibilities:
1. Conduct initial holistic assessments and evaluate care programmes for acutely and chronically unwell patients, often with complex needs.
2. Use clinical reasoning to assess, plan, implement, and evaluate patient-centred interventions based on evidence-based practice, ensuring high-quality and harm-free care.
3. Collaborate with service users, carers, and families to identify achievable goals that maximize quality of life and independence.
4. Facilitate appropriate referrals to other services.
5. Work within a multidisciplinary team, sharing knowledge and providing specialist skills supported by a competency framework.
6. Apply occupational therapy skills and techniques in assessments and interventions as an autonomous practitioner.
7. Assess and arrange equipment provision, ensuring proper functioning and fitting, and report issues appropriately.
8. Supervise team members, students, and unregistered workforce in occupational therapy competencies.
9. Maintain accurate and legal records.
10. Participate in training, including assisting in the delivery of training.
