We are seeking a Band 4 Assistant Practitioner to work within the Frailty Service at Glasgow Royal Infirmary (GRI).
This post holder would join an established team of Physiotherapists, Occupational Therapists and Healthcare support workers to deliver patient centred care.
This role will be leading the way in facilitating AHP input within our Frailty Service 7 days a week. There will be a focus on frail patients with co-morbidities to ensure early assessment, treatment and discharge planning. Input and assessment will support hospital flow and timely discharges. Working with an MDT approach to ensure the best care possible is provided to each patient.
The individual will be responsible for the independent management of their delegated caseload, from initial assessment to discharge. In the instance that patient’s needs change and become more complex, there will be the opportunity to hand them over to the team’s Occupational Therapists and Physiotherapists. It will also be expected for the post holder to make onward referrals to relevant community services at time of hospital discharge.
Good time management and confident clinical decision making, and an understanding of the role of Physiotherapist and Occupational Therapists is essential for this role. Previous experience working within Physiotherapy or Occupational Therapy healthcare support worker roles is essential for this role.
Training opportunities will be supported to carryout relevant competencies and to be involved in team service development projects. We are looking for someone who is highly motivated and enjoys being part of an enthusiastic and hardworking team.
