Overview
A vacancy at Central and North West London NHS Foundation Trust. Camden Rapid Access Services (RAS) is a physical health admission avoidance and supported early discharge service with three pathways: Discharge to Assess, Urgent Community Response, and Virtual Ward. The post holder will work across all three pathways within Camden RAS.
The role is for a Band 8a Registered Advanced Clinical Practitioner (ACP) with substantial experience in acute care (e.g., A&E, AMU, ITU, Virtual Ward, Paramedic). The ideal candidate will be a non medical prescriber V300 with advanced clinical assessment skills and have completed the ACP modules. A valid UK driving licence is preferred (car provided when required); non-driving candidates will be considered and provided with an Oyster card while on shift. Non-driving practitioners will be expected to undertake the same number of visits per day as driving colleagues.
The overall aim is to avoid unnecessary hospital admissions and support early discharge in line with the NHS Long Term Plan. Benefits of keeping patients at home include reducing Healthcare Acquired Infections and promoting ongoing independence.
Responsibilities
* Deliver advanced clinical practice autonomously in general medicine, frailty, respiratory, and cardiology, focusing on supporting patients as an alternative to hospital admission and early supported discharge.
* Collaborate with consultants, GPs, pharmacists, nurses, therapists, and healthcare support workers within a multidisciplinary team.
* Manage clinical care in partnership with individuals, families, and carers; analyse complex problems to devise innovative solutions that improve outcomes and experiences.
* Integrate care pathways and patient management across community and acute settings.
* Provide high-quality, person-centered care through advanced clinical judgment and critical thinking in alignment with the National Framework for Advanced Clinical Practice and the four pillars of clinical practice: leadership/management, education, and research.
Qualifications and Requirements
* Master’s level qualification encompassing clinical practice, leadership, management, education, and research (advanced practice).
* Significant experience in acute care settings (A&E, AMU, ITU, Virtual Ward, Paramedic).
* Non medical prescriber V300 (preferred).
* Advanced clinical assessment skills.
* Ability to work across Camden RAS pathways (Discharge to Assess, Urgent Community Response, Virtual Ward).
* Ability to work autonomously in community settings; willingness to work flexible hours as required.
Shifts and Working Arrangements
Shifts are Monday to Friday, 9am to 5pm, with potential future moves to a seven-day service. The Trust offers flexible working opportunities. Lone working is routine in community/home settings, including evenings; robust safety systems are in place.
